Prime Support Service
Prime Support Service provides home and community support services for adults (18 years +) who have learning or physical disabilities, older adults and adults who experience mental ill health. We enable people to remain in their own homes or within supported living environments hence giving them independence, privacy, choice, and control over their lives. We will operate in the Stockport, East Cheshire and South Manchester Areas. We will support and empower Service Users to design their own Person Centred Care Plans, and we strive to deliver support services tailored to meet each individual need. We provide support within the home environment and out in the community, maximising social and leisure activities to the fullest extent. We are more than just support. Our focus is on creating a client centred service, whereby you choose every aspect. Each and every person is unique and different, and it is our aim to accommodate these individual characteristics in an environment that promotes independence and improves quality of life.
