How will Reablement work?
Our Reablement Service will try to offer you a range
of flexible support in your home, arranged at times to suit you.
For some people it could be help making meals, while others may
need help to get out of the house and take part in social
activities. Whatever your needs we will work with you to
design a service that works best for you. We
will encourage you to review your progress along the way
and help you to set yourself goals.
What happens when Reablement ends?
We hope the intensive support you have received will mean you
need less support, or even no support, to live independently. We
will look at your progress to see how you are doing; some
people may need longer-term support after the service has
ended.
If you still need help after a period of Reablement, which can
be up to six weeks but in many cases is much less, we
will work with you to plan a package of care and support that
meets your needs. There may be some cost for
this but the actual amount you pay will depend on what
services you receive. You may be able to get help with the
costs so we will help you fill out a financial assessment form
to see if you qualify for help.
How to get in touch
The Adult Social Care Team is the first point of contact for all
referrals and general enquiries. You can contact them from 8am to
6pm, Monday to Friday on 0161 217 6029. In an emergency, out of
these hours phone 0161 718 2118.
When you contact us we'll discuss your situation over the phone,
check that we've got all our facts straight and then follow this up
with a visit to your home to talk about your personal and practical
needs and to discuss what help is available.

Now its your choice - and it doesn't have to be daycare! If you
want more control over your support, instead of the Council buying
your social care we can give you the money we would have spent. You
can then spend it in the way that you see fit!
