What is this?
Intermediate Care is all about local health and social care
services working closely together to support older people in a
crisis situation. This could be where they are suddenly unable to
safely undertake everyday tasks due to ill health or where
their main carer's own health and ability to carry on caring has
suddenly broken down. This service is normally for a short
period of time until a person can live safely and independently in
their own home and can help to reduce unnecessary
admissions to hospital or long term residential care.
Intermediate Care services could be in your own home, or in
another residential setting, and aim to help older people to regain
their independence by providing dedicated physiotherapy,
occupational therapy, social work and access to community
nursing.
Who is it for?
This service is for people over the age of 65 who have had a
visit in the last 24 hours from their GP or have been seen in
Accident & Emergency to ensure that there is no medical reason
for their sudden deterioration in health. Or it could be where
their main carer's own health and ability to carry on caring has
suddenly broken down.
In order to work out how much help is needed to enable you to
regain and maintain your independence, a team made up of health and
social care staff will carry out an assessment.
What is an assessment?
After the assessment we will identify and agree your needs with
you and discuss the help and support available to meet them,
according to the seriousness of your situation. If during this
period of Intermediate Care, it is decided that you need help on a
longer term basis that will also be discussed with you.
How much does it cost?
The service is free and the support may last for up to a maximum
of six weeks, but is usually expected to last between one and two
weeks.

2118.
When you contact us we'll discuss your situation over the phone,
check that we've got all our facts straight and then follow this up
with a visit to your home to talk about your personal situation and
to discuss what help is available.
