Help after leaving hospital
If you have been discharged from hospital following an illness or operation, you might need practical help with getting in and out of bed, washing and dressing, or preparing a hot meal.
If you have been admitted to a ward and the staff there
feel that you will find it difficult to manage at home without some
help, a Social Worker based at Stepping Hill
Hospital will assess you before you are discharged.
What is an assessment?
After the assessment we will identify and agree your needs with
you and discuss the help and support available to meet them,
according to the seriousness of your situation.
If appropriate, we arrange for a care worker to call daily
to support you when you return home. Or you may need pieces of
minor equipment to help you to manage when you return home. If so,
we will arrange for this to be delivered or fitted in your home, or
in the home of someone who is caring for you. These services
will help to maintain your independence and quality of life.
I need help with living at home
If you don't qualify for a care package from Adult Social Care
you could arrange to employ a care worker privately through a Home
Care Agency but remember to take care to choose one that
is registered and inspected by the Care Quality
Commission (CQC). Follow the link below for more information or
visit the CQC website.

Other help
Age UK Stockport's Home After Hospital service offers short
term practical assistance and emotional support for older
people who may live on their own and have no help or support
when they go home from hospital. This usually covers the first
few days, to help people get back on their feet.
The practical support offered could include arrangements
for shopping, help preparing refreshments or light snacks and
emotional support to help you settle when returning home. Please
note this service is unable to provide personal care.
