Stockport Council provides 3 star 'excellent' social care for vulnerable people in Stockport through a wide range of caring, advisory and support services.
Our aim is to achieve the best possible quality in social care
for the most vulnerable people. To achieve this we work closely
with local health services and other independent and voluntary
organisations to make sure that people get help when they need it
most.
Our work incorporates the following main service areas:
- Referral & Information Service
The central point of access for all general enquiries, referrals
and assessments relating to vulnerable adults and older people
We aim to fully support adults of working age who have physical
or learning disabilities or problems with their sight or hearing.
This includes involvement with equipment and adaptations where we
work closely with colleagues in NHS Stockport.
We work closely with a number of agencies to ensure that older
people are able to remain living independently at home and have the
ability to exercise choice and control of the services that we
provide or arrange for them.
Working in partnership with Pennine Care NHS Foundation Trust we
aim to support people with mental health issues including drug,
alcohol and substance misuse.
