How to apply?
Phone 0161 474 3422 and leave your name and address and we will
send you an application pack. Or you can pick up an application
form from any Stockport Direct Local Centre or library. You can
also get an electronic version of the form to print off, and the
guidance notes by following the link below.

If you believe you are automatically eligible for a badge,
please complete the form and send it back along with a current
letter of entitlement from an awarding
body. Please enclose the proof of address and identity
and two recent passport style photographs with your name signed on
the back. The following are acceptable:
1. Confirmation of address showing applicants name
You will need one thing from this list:
- Council Tax bill
- Utility bill
- Credit card statement
- Bank statement
- DLA letter.
2. Confirmation of identity
You will need one piece of photo ID from this list:
- Photocopy of passport ID page
- Photocopy of DVLC licence photo card
- Photocopy of Bus Pass photo card (new style from October
2008)
- Photocopy of leisure key card
- Photocopy of work ID card.
3. Confirmation of Disability Living
Allowance
You will need a current letter or entitlement, this could be either
of the following:
- An up to date letter obtained from the Disability Living
Allowance centre, telephone number 0845 7123456 or
- The original letter of entitlement plus a copy of your annual
payment notification letter.
How long will it take to come?
New applications: If your application is
successful you should receive your badge within 10 working days.
However, due to high levels of demand at the moment you may
experience a delay in response so please allow up to 21 days for
your application to be processed
Renewals: You will receive your replacement 3 or 4
days before the expiry of your old badge. Please do not telephone
to enquire about your badge before this time has elapsed, to allow
for postal delays.
What you need to do
To apply please complete the form and take it to your GP along
with the proof of your address, proof
of identity and two recent passport style photographs
signed on the back.
Your GP will complete the forms and return them directly to the
office specified. Please allow 10 working days for a response to be
received by ourselves from your GP. We aim to deal with your
application within 10 working days of receipt from your GP but due
to high levels of demand at the moment you may experience a delay
in response so please allow up to 21 days for your application to
be processed.
If you have not had a response about your application after four
weeks please call your GP surgery in the first instance to make
sure they have returned the information to us. If they confirm they
have returned the information more than 10 days ago, please ring
0161 474 3422 between 2pm and 4pm only to make sure we have
received it for processing.
On receipt of your Blue Badge and supporting booklet please make sure you read the information about how to use it properly. The badge should be kept safely when not in use, preferably not in the car. We receive many reports of cars being broken in to and badges stolen.
